DUTIES AND RESPONSIBILITIES
- Caseload of approximately eight (8) patients who will remain in residential treatment 60-120 days, depending on clinical needs.
- Complete bio-psycho-social assessments for each client and develop treatment plan.
- Counsel clients or patients, individually or in-group sessions, to assist in overcoming dependencies, adjusting to life, or making changes.
- Attend weekly Case Conferences.
- Act as a liaison between clients and other service agencies involved in clients’ treatment, such as Probation, Parole, DCFS, etc.
- Facilitate weekly group (average 2-3 a week)
- Support of clinical team to access resources for patients (i.e. therapy, care coordination, peer support specialist, etc.)
SKILLS AND KNOWLEDGE REQUIRED
- The ability to document client activities and / or carry out such written assignments is required.
- Experience with PCNX (formerly known as SAGE)
- Experience with Medically Assisted Treatment (MAT)
EXPERIENCE AND EDUCATION REQUIRED
- Certified Addictions Counselor, in good standing with a certifying organization (CCAPP, CAADE or CADTP)
- Minimum 2 years experience in field of drug and alcohol treatment.
- Computer experience and good verbal and writing skills.
- Reliable transportation.
- Professional appearance.
- Must have valid drivers license.
- Must be vaccinated and have Booster for covid or comply with mask requirements
- Must be eligible to work legally in the U.S.
