Position Description
Responsible for the administrative interpretation and evaluation of test and measurements, plans and modifies treatment program, works with young adults through geriatric individuals with clinical competency, completes established competencies at Kirkland Ministries Inc (KM-Inc.), provides care to intellectually and/or developmentally disabled individuals in a residential and/or in-home setting.
Position Duties
Physical Therapist duties include, but not limited to:
- Interpret physician orders and requests interpretation as necessary.
- Administer and interpret tests and measurements of patients.
- Review patient chart and progress prior to treatment and modify the treatment plan accordingly.
- Develop realistic functional goals based on patient and/or family expectations.
- Instruct and involve patient, family, social worker, nursing staff and discharge planner in patient’s progress and discharge plans, participating in appropriate meetings.
- Maintain documentation on all treatments.
- Use appropriate modalities and procedures correctly demonstrating knowledge of indications and contraindications.
- Assure appropriate equipment performance prior to each use.
- Organize schedule to reflect patient priorities, needs of other staff, and the department needs and responsibilities.
- Provide in-service to staff
- Provide clinical education for PT students.
- Utilize supportive personnel and supervises administration of treatment plans.
- Inform supervisor of incidents, accidents, equipment failure or unusual physician or patient problems immediately, initiating appropriate action.
- Maintain a working knowledge of routine clerical procedures.
- Utilize resources efficiently and effectively.
- Maintain safe environment
- Address patient population –specific needs
- Participate in Performance Improvement activities.
Requirements
DCS candidates must meet the following requirements:
- 18 years or older
- Master Degree in Physical Therapy
- Documentation of proficiency evaluation of experience and competence including, but not limited to:
- a written assessment to ensure the candidate’s ability to document services delivered of the residents to be served
- at least three written personal references from persons, unrelated to the candidates, that indicate the ability of the candidate to provide a safe, stable, and healthy environment for residents being serviced.
- Have at least three years of Physical Therapy experience.
- Certified copy of a three-year driving record from the DMV.
- Ability to pass a drug screen, physical exam, and a criminal background check.
- Ability to work in a residential and non-residential setting.
Required License or Certification
DCS candidates must have the following,
- Valid Driver’s License
- Current Automotive Insurance
- Current Vehicle Inspection and Registration
- Current Physical Therapy Certification (state exam)
