Social Worker Position Details

- Position Pay: $20.00 – $ 30.00 per hour + travel reimbursement
Position Duties
The Social Worker’s duties include, but not limited to:
- Support the Collaborative Case Management Model.
- Demonstrate an understanding of funding resources, services, and clinical standards and outcomes.
- Demonstrate mastery of social work practice related to assigned population(s) to include: protection of at risk populations, cultural boundaries, knowledge of social programs providing.
- Demonstrate a good understanding of managed care trends, Medicare, and Medicaid regulations, reimbursement and the effect on utilization of the different methods of reimbursement.
- Demonstrate the ability to develop departmental interfaces with internal and external customers to provide exemplary service and achieve goals.
- Implements and oversee implementation of the transition.
- Collaborates with physicians, office staff and ancillary departments.
- Complete concise documentation in Case Management systems.
- Maintain accurate patient demographic and insurance information.
- Maintain objectivity in decision-making, utilizing facts to support decisions.
- Support the mission statement, policies and procedures of the Kirkland Ministries Incorporated.
- Achieve ongoing compliance with all regulatory agencies.
- Demonstrate flexibility in response to change.
- Anticipates and respond to problems and risks.
- Communicate effectively with all levels in the organization.
Requirements
Social Worker candidates must meet the following requirements:
- 18 years or older
- Master’s Degree in Social Work
- Documentation of proficiency evaluation of experience and competence including, but not limited to,
- a written assessment to ensure the candidate’s ability to document services delivered of the residents to be served
- at least three written personal references from persons, unrelated to the candidates, that indicate the ability of the candidate to provide a safe, stable, and healthy environment for residents being serviced.
- Have at least 2 years acute care experience.
- Certified copy of a three-year driving record from the DMV.
- Ability to pass a drug screen, physical exam, and a criminal background check.
- Ability to work in a residential and non-residential setting.
Required License or Certification
Social Worker candidates must have the following,
- Valid Driver’s License
- Current Automotive Insurance
- Current Vehicle Inspection and Registration.
- Licensed by the Texas State Board of Social Work Examiners.
