Selected partners will receive tailored support from nationally-recognized leaders in trauma-informed group health coaching. During this 12-month program, partners will receive training and support to design & launch a clinically-integrated, trauma-informed, transdiagnostic health coaching program (or strengthen an existing one).
The Community as Medicine Learning Collaborative includes:
- Tailored support for your organization to build or strengthen equity-centered health coaching programs leveraging practices from Open Source Wellness’ evidence-informed and nationally recognized Community as Medicine health coaching model;
- Group and 1:1 training and capacity building to address programmatic challenges such as clinical integration, optimization of referral workflows, session design, data and evaluation and more;
- Strategies to make your health coaching program financially sustainable, including innovative revenue sharing with group medical visits, CHW/allied workforce billing and other approaches;
- Reserved spots in Open Source Wellness’ National Board for Health and Wellness Coaching-approved health coach training program that specializes in cultural humility and trauma-informed practices. This 5-month training program prepares individuals to become Board-certified health and wellness coaches;
- Participation in a cohort of YMCA and community partners that are building equity-centered health coaching programs and are committed to sharing experiences and lessons learned; and
- Access to an integrated curriculum library, evaluation surveys, and other resources, as needed.
Dates: January - December, 2024
Cost: Free for selected partners. Open Source Wellness is offering its Community as Medicine Learning Collaborative free of charge to 3-4 organizations in 2024 due to generous funding from The Ardmore Institute for Health and Prime Health.

Commitment:
- Dedicated staff time to 1) attend training and capacity building sessions; and 2) design & launch a new health coaching program (or strengthen an existing one). We anticipate the required time to be 4-6 hours a month on average, but will depend on team and individual needs. Teams with multiple staff members participating will be prioritized to ensure program continuity.
- Intent to deliver the health coach program for (at least) 1 year post-launch
- Willingness to evaluate and share program impacts with measures to be determined in collaboration with Open Source Wellness
